Introducing Care Connect Corps

A Patient Navigation Tool

The “Care Connect Corps” is designed to empower medical students, GME trainees, and attending physicians with a tool to provide outpatient care coordination support to patients experiencing housing insecurity before discharge from the GW hospital. The goal of this tool is to help prevent what we have termed the “Never Event” of social medicine: the belief that discharging a housing-insecure patient to the street without coordinated outpatient follow-up is not merely a “difficult discharge” but a preventable systemic failure.

We have distributed badge reels to third-year and fourth-year medical students.

These badge reels include a QR code to a patient navigation tool that allows navigators to… 

  1. Schedule primary and specialty care follow-up at the bedside
  2. Coordinate meds-to-beds medication access
  3. Connect patients with support for Medicaid application renewal or submission
  4. Assist patients with initiating transportation access to their appointment

While our organization focuses on serving people experiencing housing instability and homelessness, this tool can be used with most patients to streamline the discharge process and connect them to the care they need.
